This form may be submitted to appeal a library fine, fee or replacement cost.
Fines/fees are generally not waived due to:
• Lack of knowledge of library policy
• Non-agreement with library fine or fee structure
• Material loaned to another person
• Non-receipt or late receipt of library reminder notice
• Being out of town
• Forgetting the due date
• Not being able to get to library to return the item
• Financial hardship
PLEASE NOTE: Hourly and daily late fees do not actually show on your account until the late item has been returned. Note that if you are a student, library charges that have been transferred to your student account will no longer show on your library account.
